Maybe people are imagining the median to be larger than actually pictured. If the median is so wide that you can fit multiple cars in one direction on it and it's basically a cross road with lane markings and everything, then you stay to the right. If the median is only wide enough to fit one car and there are no lane markings, it's basically just part of an intersection. When you're turning at a left green arrow do you try to cut across the path of traffic turning left in the opposite direction? No, because that's ridiculous. That's the kind of intersection pictured.
I was taught (in drivers ed like 30 years ago) to creep out into the intersection so that when you get an opening you can make your turn faster without surprising anyone.
Also, around here at most standard 4-way intersections they don't give traffic in both directions a green arrow at the same time. The will stop traffic in one direction, then give a left green arrow and a green light for the traffic in the other direction. At some point the green arrow turns to red and now both directions have red arrows but green lights for going straight. Eventually the straight traffic that was going first gets a red light and the oncoming traffic which already had a green for going straight keeps going straight and also gets a left green arrow.
I can think of 2 large intersections here that do give lefts at the same time. Both have painted turning lanes and both route the traffic inside (to the left) of the opposite turning left traffic. (So basically what the picture labeled as wrong and what you guys say is correct.)
Also, I did a web search and found plenty of examples that say I'm wrong for standard intersections. So we're obviously ******ed up here in the Northeast. Or I'm ******ed and somehow have never managed to hit anyone.
