Took me 300+ hours to unlock everthing in Rebirth then in Afterbirth, combined expected should be probably much less (can't explain why without spoilers). Personally unlocks are always what drives me playing in Isaac, I'm having a blast for 200-300 hours unlocking everything, then once it's done I just quit and have no regrets.
Also I think this is pretty accurate relative to the increase in difficulty, I played tonight after work and it didn't seem like the difficulty spiked a great deal. Depending on the items you get off the bat it was about the same. Although if there is one really basic enemy I could get rid of it's those goddamn spiders. They are so erratic and I'm still terrible at dodging.
I did, for the first time, get two item rooms and two boss rooms in the basement. I assume that's just a random thing that can happen like 1% of the time or something? Was excited to see two item rooms, then fought boss 1 and was like wait, there's no trap door, just another boss room door. Forunately one of the bosses was one I consider super easy at this point.
