The "heads" are guests of US "allies" (Turkey, Qatar, probably Egypt) where Israel is not allowed to go after them. On the ground in Gaza much of the head of the snake has been cut off in the last 7 months, which is why Hamas is so ineffective as a fighting force or doing much more than stealing aid and shooting Palestinians.
But there is a lot of guns and a lot of people willing to shoot them (mostly at Palestinians while stealing their food), so still a lot of Hamas. I mean if the objective is to just hide in tunnels, poke your head out every once in a while to take pot shots at Israeli soldiers, and steal all the aid after the soldiers leave, that isn't too difficult a strategy that small cells couldn't do this on their own without much leadership or coordination.
