Worldwide health insurance

My current travel (and health) insurance is about to run out and i am looking to get a new policy, however it looks as though you can't begin a new insurance policy if you aren't currently in your country of residence.
I am in thailand right now and have no plans of returning back to the UK for quite a while but i want to make sure i have at least health insurance in case anything big happens.
Can anyone recommend any companies/policies that you can start when you are already away?

Doesn't Thai visa offer insurance? That is if your planning on staying here.

There are quite a few for expats, but I wouldn't count on actually getting my money if something serious happens as they're almost all brokers of offshore based insurers.
Hospitals are rather cheap in thailand, so unless you fear serious illnesses you'd probably be better off saving the money an insurance would cost you and paying out of your own pocket if you happen to break an arm or whatever.

I need something like this too. I remember reading the T&C's for the ehealthinsurance policy that I had in California and the coverage for just being out-of-state was ridiculously low/bad.
