being unattractive is the biggest thing but a lack of social competence and unwanted sexual advances (especially the small / stupid things) all lead to it imo.

if you're unattractive you're simply that. sure, that affects how people receive you but it's pretty different from being called creepy.
Wasn't there some sort of saying, that if you walk into a room and after ten minutes you don't recognize the creepy person there, then it's probably you?
No, not if you talk to males a similar amount. I find men who really, really like to talk to women very odd and a little frightening...
What about men who like to throw their pokeballs at you?

Mainly two things: behavior that is incongruent with the social atmosphere and appearance of an ulterior motive with negative consequences (from the perspective of whoever is judging).
To rephrase it, when you're being weird in a way that makes others feel fearful of what you may be trying to do.

It also depends on the person. Some girls I've met have called people creepy when they (the girl) misread the other person. I mean, they will usually not consider that same person a creep when they notice he isn't, but there's definitely some girls who tend to over-label people as creeps.
In general though it's just someone who makes a person uncomfortable due to what they say or body language. Also why is this a thread ? It's not really that hard to notice.
a lot is based on looks, some of it is tone. A big factor is emotional intelligence.
two guys can say the exact same thing and one can be creepy while the other is funny.
if you're talking about guys flirting with girls... if the girl is sexually attracted to the guy, not creepy. If the girl isn't sexually attracted to the guy, creepy. generalization.
Being creepy or sexy entirely depends on what the girl thinks of you at the time.
You can check your girlfriends' iPhone while she's not looking, find out her most played song, learn it for her on the guitar, and play it for her sometime. And it's either "omg that's soooooo cute you're amazing! xoxoxoxox" or "what the **** you looked at my phone" and get ignored for days and broken up with.
Girls project their own insecurities and just are generally messed up.
There is nothing that inherently makes a guy creepy (unless we're talking ugly old men who actually stalk women without talking to them and that ****, but that's not the discussion here,) it is entirely the woman's interpretation of what's going on that makes her believe it's creepy or attractive.
