Just picked up a script, 6 pills for $122, or 20 bucks a pill. I understand viagra is about the same. I am in the US and dont believe in insurance so what options do I have? Suck it up and pay? Can I get this pill online abroad safely? Anyone know of any generics of alternative medicne for this issue?

If your scrip for Viagra is for 100mg half of one is enough to get you going. That halves your price fwiw. Personal experience and such. I have insurance though so I'm not sure how much they cost. I get them for free.

"Don't believe in" carries a connotation of like moral, principled opposition. Or disbelief.

Either of those are really dumb for someone spending $120 on SIX pills. Even if half of one gets the job done that's $10 every time you have sex.

Health insurance is a bad purchase for a healthy mid-20s self employed person. If you're 38 and need prescription medication?

I can understand the opposition to something like car insurance where the price of fixing stuff is generally the same whether it's being paid out of pocket or by an insurance company. Without health insurance you just get ****ed in the ass unless you haggle everything (see OP).
