Odds Oracle- Difference between "Hand vs Hand" and "Hand vs Range" Graph?

Hi,
can anybody tell me according to which mathematic algorithm these two different graphs are constructed and what the meaningful differences are? All I read on propokertools.com was that the the difference is that the "Hand vs hand" Equity graph (for the next street) is constructed based on faced up hole cards and the "Hand vs Range" Graph works with hidden hole cards but I don't understand what this means exactly.
I can imagine how the "Hand vs Range" Equity Graph is constructed. It should be in the following way:
Run Equities from all individual hands out of the Range vs the one particular holding on specific flops and then just take the average equity. Let's just assume the calculated Equity on one particular flop (5s2cKd) is 50% and since the specific flop is x% of all possible flops one stat for the graph will be: x% (Frequency) * 50% (Equity)
But I have no idea how the "Hand vs Hand" graph is calculated. Can anybody explain it to me?
