Is there a lag compared to realtime for the players playing on Twitch and streaming live?

There's an automatic lag of about 30 seconds and you can build in a lot more by design through playing with the settings.

It also probably works better on sites with anonymous tables. Or, and I work there, Unibet due to alias changing and no table selection.

The ones I have watched are on delay (usually 3 minutes)

I am not sure I would call it a lag
The person operating can see chat real time, but the chat is frequently in response to
what they are seeing on the screen that happened XX minutes ago.

and the speakers response to the chat will not be heard for XX minutes.

Speed of light delay

The streaming software has a delay setting, in addition to the 30 second inherent latency of encode/upload/download/decode. Most folks set it to 3 minutes but some Zoom players leave it at zero and live dangerously.
