Full Tilt tournament hand history problems

I've searched and searched for an answer to this question, to no avail . . .
I save hand histories for import to PT3 and have noticed that for about every tenth tournament, the hand histories stop being saved to the file (and therefore don't get imported to PT3). It's clearly not PT3's fault, since the missing hand histories never make it to the tournament file in the Full Tilt directory.
Has anyone else seen this? Any idea what I might be doing wrong?
