Well, I don't fold fifth unless the other guy is a known quantity. There is enough chance that he has a straight or at least beats the lone pair that you're representing. When I check the river, it is with the intention of trying to pick off a bluff.

yeah what andy said. with the check 6th, it seems very likely to be a "free-card if i miss" raise on 5th with 4 hearts. I try to snap off the bluff as well on the river.

He could have just hit a 4 card flush and is now raising.. Maybe a pair and flush draw, maybe a straight, maybe 2 pair, maybe just splashing it around.. The only hand you should fold against is made straight (or 3 of a kind).. I call 5th/6th.. Calling 7th depends on what hits on 6th.

Check/Calling the river seems fine, especially against a bluffer. Against weak players I prefer to bet the river and fold to a raise.. Obviously this line is bad against a guy who likes to bluff since you aren't giving him the opportunity, and if he bluff raises it really sucks.
