I just had mine this morning. They just check off stuff by rote. Essentially just circle "agree" to all of the statements.
"The nurse demonstrates competence in XYZ" - AGREE
Only she did ask me if I really had to give Ativan to the combative guy I couldn't manage with security and 4 nurses (two of whom were male and fit). Apparently he was sleepy the next day and I should work on non medication ways of calming down people that are breaking through their restraints. Will do.
Damn girl, X, Y AND Z? you must be a heluva nurse. I think I'd give ativan to every patient that rubbed me the wrong way in small talk. I admire your profession quite a bit tho

Thanks. It's really easy for administrative people to talk when they don't have to manage the situation on hand. I thought I was a boss for getting through that night without him falling and without any staff getting injured. I mean the security guy and 3 of my colleagues can't be dedicated to him all night, so when I asked her what my better options were she didn't have much to say.
