Happens every year. Not necessarily just nerves though for sure that can account.
Courses can be really hard and long and may not compare to the course the qualifying occurred on. For example, a guy could play a 6600 course in sectionals and then get there to Atlanta and the course may be playing 7400 yards (haven't looked at actual yardage). So there are a lot of guys that could potentially shoot 140 or better in qualifying and perhaps on the 7400 yard course he may have a hard time breaking 80. Greens/pin positions may often be much much tougher too.
Add in loss of swing or putter in the month since qualifying and its just not hard to imagine a round spinning out of control with no ability to get back on track.
