You get it during your second extension. So you arrive, get a one month visa on arrival, after a month you extend for another month, then after two months being in Phils in total you can get the ACR card. Actually, you can get it faster than that if you want, but you must extend your visa first.

AFAIK you need to go to Intramuros for extensions. I went to the Makati office for my first extension and they just directed me to Intramuros.
