i think you are in the wrong threads if you are putting malaysia in front of thailand on anything. the only thing they are good for are visaruns and not even best place for those.

i remember i took a motor bike for 5 mins and the guy randomly charged 170 baht. i only gave him 120 and left, but then i met a 2+2er and he sad it should only have cost 40 baht. not that it matters all that much, but they try to nickel and dime you so much, just trying to not get ripped off as much as possible.
from kamala to patong in tuktuk i would say 200-300
from kata to patong in tuktuk 400-500
you can get to the big buddha from chalong. drive to chalong circle. take the first left if coming from kata/karon and just look for the signs, shouldnt be more than 2km from the circle
always get a quote for fares with taxis/tuktuks/motorbiketaxis in phuket and haggle. put in a few words in thai and smile, never raise your voice.
only metered taxis in phuket are FROM airport to place x and sometimes even they try to scam you

Motorbike guys usually have their fees listed in thai on some paper right where they hang out, which gives you a rough idea of the pricings, but generally shouldn't be more than 50.
even after they say they will use the meter make sure they put it on. if not, change taxis.

like others said already a trip like this sounds like hell. i wouldnt make a minute to minute timetable for A HOLIDAY. i wouldnt want to see more than one place a day and then relax the rest of the day. all you would do on your trip is travelling and taking pictures. if that is what you are into then please, go ahead but if you are inexperienced traveller then you will notice pretty soon what kind of trip you are into if you try to see all those places.
