Common reasons for failing a lift:
-Not strong enough for the weight.
-Not recovered (rested/fed) enough for that session.
-Form sucks.
-Other life stress factors weren't left at the door (not concentrating on the lift).
-Weight felt heavy and you b!tched out.
Which one of these, or numerous other issues, is the cause for you failing a lift is basically impossible to figure out over the internet without further information or video.
