Most people that use a pussy pad do it because they don't know how to tighten their upper backs properly and use their muscles both as a cushion for the bar to keep the bar from being in contact with the spine and to keep the spine in a safe, neutral position. They typically let the padded bar rest on their rounded spine, which is harmful - now they have a pad digging into their rounded spine, so that hurts less than the metal bar with no pad, but its still obviously bad. Its harder to get a straight bar in a good, safe position with a pad on it, than without a pad.
People that are good at squatting know how to hold the bar properly with no pad.
