MP limps, CO isolates 4bb, SB(hero) with A7s ???

In real game, actually hero just flats, fish flats and luckily hit nutflush draw and win the pot. Even though MP is a total fish and getting an additional value is more comfortable it is too uncomfortable to be in total oop in a 3way pot. (actually I didn't play this hand) So, not just this A7s but also other mediocre hands like A5s, KTs is it better to just flat? Or raise/fold is optimal?

I think if you are gonna play with an ace, it's much more profitable to raise with it. Flat calling in the small blind just for the sake of 'pot odds' in seeing a flop will just drain your money in the long run. I'd be raising in the small blind with a7 if im faced with a limper. I'd probably call a raise against a loose passive also but not all the time

If you have to ask this question, you should be playing the lowest stakes available to you at your site, not 200z unless you are a very wealthy person who is fine with lighting $100 dollar bills on fire.
