This one is a lot closer IMO and I def would not press the button in order to change the course of the train. There is no EV to be gained by pressing the button, either way there is one human life lost.

If for example a car drove over the guardrail and into the water, and you only had time to jump in and save either the baby or the 40 yr old man, I would def save the baby, but in that situation the man is going to die anyways.... In the other train example, if you press the button 5 lives are being saved, in this example one person dies regardless. I would not take the life of an innocent man who was otherwise not going to die simply to save the baby....

That's why I think it's closer @ least, I hope I did a reasonable job explaining it....

I must be missing something here because as far as i can see, its a choice between sending a man who is 40 yrs closer to the grave than the baby to his demise.

No it isn't. You are actually killing a man who wouldn't otherwise die.
If a kid needs an organ transplante you don't just kill older people to remove their organs, right?
