Bryant and Harper are exceptions to the rule. Basically players with less than 3 full years can have their contracts renewed by their team for the league minimum or more, and then players with more than 3-> less than 6 years control go through the arbitration process.
Some teams have policies or formulae that determine what the salary is for 0-3 guys, others just kind of pick and choose. Can be based on service time, performance, etc. Cubs gave Bryant 1.05MM because he was MVP last year, but they would have been entirely within their rights to renew his contract at $535k if they felt like it.
Teams will usually offer their players a contract and the player has the option to accept/reject, and they almost always accept since all rejecting does is silently protest and ask for more money, but at the end of the day the team has all the power. Mets just renewed Syndergaard's deal at $605k. He thought he deserved a bigger raise, so he didn't sign the contract that he was offered, so they just said OK, here you go have what we offered you.
