I'm American living in the land of ladyboys Thailand.
Mainly the butch ones go to the one with the smallest line and the flaming ones go in the girl's one.
Never talked to a Thai person that really gave it much of a 2nd thought and it's the least of people's worries here but also the ladyboys don't have a real danger of getting their a** beat by some redneck for dressing up like a woman which I think could be of real concern in plenty of places in the states.

Fair enough, most family houses in the US don't have urinals of any sort.

You youngsters crack me up. Pretty much every major league baseball stadium I ever visited had a giant trough for a pisser - Dodger stadium, Wrigley, Comiskey, etc. - with a solid 10% chance of seeing some ****faced guy stagger and fall, ending up with both hands in the trough supporting him.

There are a couple of theatre's and caberet's in NYC I've been to that had unisex multi stall bathrooms.

I've also been in Broadway theatre's where they throw women into the men's restrooms during intermission. They did have a curtain that kind of separated the urinals from the stalls.

Heh - London theaters are ridiculous at intermission - I have to assume they were built back when women were supposed to stay at home because they seem to average one toilet per 800 customers. Last trip I walked into the Gents, pissed and walked out - my wife was about 100th in line, and we missed about 10 minutes of the second act....

Yep, the Kingdome had those as well.

As far as multi-stall non-gendered restrooms, the fancy Starbucks "Roastery" here in Seattle has one. Seemed okay to me. The way a lot of these are being implemented is that the stalls have full, floor-to-ceiling doors, so there's plenty of privacy.
