I trained at Northwestern in Chicago - on the medicine service we started on the 4th floor and worked our way upwards. On 17 ( I think) was the high roller rooms - looked like Bellagio hotel rooms with all the med equipment cleverly hidden behind sliding panels - it's where the rich and famous went in Chicago to die. We joked that the motto for the operation should have been something like "Near a major medical center" because by the time we got there we were worn out and disinterested.
CT is the test of choice for sinus stuff, if for some reason you want to get one. Generally, you shouldn't unless you're looking for sphenoid sinusitis which is deep in the skull and potentially lethal. Frontal/maxillary sinusitis should be diagnosed easily by anyone beyond an intern by history and exam. How helpful antibiotics are for sinusitis is controversial, although from what I can tell evidence suggest that smokers benefit from antibiotics more than non-smokers.
Generally, if you're yorking up yellow/green/brown crud, and it lasts for more that a week or two a course of antibiotics seems reasonable. If it's clear/watery stuff, probably not. On occasion, people grow ****ing fungus balls in their sinuses, with all sorts of potential disasters possible. You can't actually "swab" a sinus in an office/ER anyway - the openings to the sinuses are tiny, and you'd just end up with a contaminated specimen from the oral flora anyway.

" - this is absolutely correct. It's also helpful to do what the goddam doctor tells you to do, also.
Lastly, if anyone is using those Neti pot gizmos, for God's sake use sterile/distilled water, not tap water - some poor ******* did, and got their brain eaten up by an amoeba recently.
