There are places outside the United States where "The Americas" or "America" is used to refer to the North and South American continents collectively. This nomenclature leads to the resultant use of "American" to refer to someone who lives *anywhere* in North or South America. Hence the comment about Nicaragua.
A Nicaraguan friend in high school tried that argument to which I responded that despite there being some superficial merit to her point, the fact is that the common use of "American" both inside and outside the United States is made with reference to citizens of the United States of America. Hence the comment about "down, down, America" not being directed at Nicaragua, or Peru or Argentina for that matter if you think I'm picking on Nicaragua.
If that ruffles feathers of people living elsewhere in "The Americas".....<shrug>
