Left turn must always yield unless they have a green arrow. It doesn't matter which lane they or the other car end up in. If you're turning left and you don't have a green arrow, yield. Pretty simple.

No idea what crazy driving laws other states have.
You are always supposed to turn into the lane nearest you. This is disregard most of the time by almost everyone. I would never trust someone to follow this rule. So I always yield when I am the left turner and keep an eye out for idiots when I am the right turner. If the choice is being right or not crash, I choose not crash.
