I’m a psychologist at a hospital. Psychiatrists are certainly trained in talk therapy. Some are quite good at it. Some are not. Depending on the setting, a psychiatrist may focus exclusively on med management while having the psychologist do the talk therapy. Or the psychiatrist will provide both. Where I am, we provide the talk therapy and psychiatry focuses exclusively on med management. This isn’t because we are better at it, necessarily. It’s more about resource allocation. Psychiatrists often have several patients they may see every hour and can not afford to give the time necessary for talk therapy to their patients. However, in a private practice, a psychiatrist is better able to allocate enough time to their patients to provide talk therapy and may choose to provide it or not.

I will say psychologists have taken more classes on various treatment modalities (most of which I don’t use) and have also taken supplemental classes on psychopharmacology so we are aware of the effects of medications our patients are on. Psychiatrists are MDs that specialize in mental health. So the focus of their training is primarily medication oriented supplemented by classes and training in psychotherapy.

Research shows Social Workers who provide individual therapy have almost the same success rates as more highly trained psychologists. It seems what’s more important than formal training in successful treatment outcomes is bed-side manner, rapport, and empathy. So if people need therapy but can’t afford a psychologist or psychiatrist, I highly recommend trying an LCSW.

Incidentally, it is sadly true that we have a great number of psychologists who stink at what they do. Most credible schools try to weed out the bad ones while they’re in school. They don’t want the “quacks” to graduate and damage the reputation of the school by giving bad therapy. At my school, we had about 20 students start and I think 8 of us graduated. The others were kicked out or dropped out with a mountain of student loan debt and no degree to show for it. Sadly, there are some schools that are essentially diploma mills that just churn them out. I’m convinced it’s mostly these ones who give us a bad reputation. That’s my opinion, anyway.

chiming in as a consumer of these services, I assume that the original poster was trying to point out that you have to be careful with psychiatrists, because they can tend to medicate first and ask questions later.

Just as surgeons tend to propose surgery as an initial solution, psychiatrists can be awful with over-medication. Probably the bad ones that the doctor above described.

also, I second the recommendation to see a licensed social worker. when it comes to therapy in general, be prepared to shop around. a personal recommendation is usually a good place to start.

I got a lot out of my therapist. But at some point I felt like we weren't really progressing. I was still doing some of my same stupid ****, and just using her as a venting session every week.

I think fundamentally there's a bit of a conflict in that she has two goals: 1) help improve on my issues, 2) keep me coming back as a source of income. When maybe at some point what I need is someone to tell me that we're not accomplishing anything here if I'm gonna keep doing to same old ****.

I'm not stating this very well - but hopefully you get the idea.

Good ones usually have no problem picking up new clients. What stopped you from speaking up and saying you weren’t accomplishing anything since you kept doing the same thing? In therapy, there’s a rule that the therapist should never work harder than the client. If you want to change, that’s on you. You make that decision. You make the move. The therapist is there to guide you, help you get back on track when you need it, and support you.

That being said, if your issues were resolved and she kept you on for support sessions even if you felt you were ready to go, that’s not good. However, if you weren’t making changes and you weren’t progressing, she was likely waiting for you to speak up so she knows you’re ready to actually change. She may not have wanted to push you where you weren’t motivated to go yet.

It’s a lot easier than you would think at first glance. Like most things, once you’ve got that first time out of the way, you’re far less anxious about it from that point on.

Yup, the bolded. But make sure it's wrapped up. I've witnessed way too many guys become a dad at a young age.

And speaking of kids, don't have them. They are expensive and it's no guarantee they will be there to wipe your butt when you are 85 years old and close to death.
