I hear you. I do try to be sensitive to this issue, and I’m pretty sure I’m OK with my dentist. This thread is the reason I asked him, and my read of his response was that it was genuine. In my office, I don’t keep my shoes on when I’m inside my own personal office, but I almost never even step into the hallway without shoes on. The exception might be if it is late at night, nobody else is here, and I need to walk ten steps to a large color printer.
With respect to driving, I believe a fair number of people drive without their shoes on. Women who wear heels, for example, routinely take them off behind the wheel. Either way, I don’t really see the objection to doing this. What the hell is the problem?
