Most of the time it is because a patient is set to be seen for one symptom and then when they get there they have 5 others they want checked out or they come to a walk in clinic or make an appt with their family physician for something like chest pain or left side weakness when they should have gone to the ER or they are not honest when asked what they would like to be seen for until they get in the Dr's office and then they have a whole other reason they want to be seen.
Most medical back up is caused by the patients themselves. We don't just stand around staring at the wall just so we can make people wait because we love getting bitched at all day by sick people.
Also if you go the ER and 4 people come in with chest pain and you are there for a fever with stable vitals, well then you are just gonna have to wait and if someone comes in after you throwing up blood then you are just gonna have to wait some more.
