This is the biggest reason winning online players go broke I'm sure. But how common is this among live pros playing at least 5/10? You hear about live pros going broke. Is this the most common reason or is it more likely they've just blown it on hookers and blow?

I read Barry Greenstein's book "ace on the river" and he said before he learned bankroll management he was down $1 million and he also said that a lot of very good players are capable of winning lots of money but they do not know how to manage it.

I'd say it is really common. I think this is even more true for Online to Live crossovers, because they can beat games so much higher than the games they're rolled for.

I read a lot on how "pros" took a shot at the next level and did very well. I hear that more than I hear that others took a shot and failed and had to go down a level. (at least by players who admitted it)
I've taken enough shots in my career to know that I don't need to be taking shots. But I'm not a pro, so who gives a crap what I think?
I would not be suprised if many tournament players who are rolled for cash games are adequatly rolled for $10k+ buy ins. They may be rolled for satellites but not the larger buy ins.
