There was a new update earlier today but everything is working on my end.
Very occasionally if you are logged in and there is some kind of disconnection, the system still reads you as logged in and you'll get that "Unable to Connect" error. Restarting the system cures this the majority of the time as basic as it sounds. The rest of the time a quick msg to support via Live Chat etc. and they can manually remove you from the system.
If you hadn't been logged on yet and were downloading the software for the first time, my best guess would be that your firewall is either preventing the download/install from working fully, or the software from starting up. Have you tried running without it just to be sure?

Not to mention I've made several cashouts from poker sites without fees.
One of my banks charges $25 on every international wire transfer that comes in, regardless of the amount. So that is entirely possible (at the very least worth looking into to make sure).
Also you may have had a fee for over drafting the account.
