Ok I have a question...Why is it that there is ALWAYS either a raise or fold preflop in these heads up matches. There is never any limping. Is this standard for all HU cash games like this?

Isildur will only play under the stipulation that there is no limping or min-raising.

Some players do incorporate limping the button in their HU game (see: David Benyamine, Omgclayaiken, David Benefield), but I guess Ivey doesn't very often and Isildur seems to find the very thought of it offensive ;P
