It appears 2-year cost for the ATM card are:
Card - $80 (fixed)
Monthly Fee - $4.95 x 24 = $119 (fixed)
Loading Fee - $25 (Can load up to $2500 at a time)
So, if we set tiers to the expected amount we will withdraw in that 2 year span, % cost of total withdrawal is below (based on $500 withdrawal at a time via ATM, $2500 card loads, & ~$5.50 ATM fees each transaction for US players):
$200K = $2,000 loading fees + $2200 ATM fees + $199 fixed fee = 2.19%
$100K = $1,000 loading fees + $1100 ATM fees + $199 fixed fee = 2.23%
$50k = $500 loading fees + $550 ATM fees + $199 fixed fees = 2.49%
$25K = $250 loading fees + $275 ATM fees + $199 fixed fees = 2.89%
$10K = $100 loading fees + $110 ATM fees + $199 fixed fees = 4.09%
Gets to be pretty steep if you don't expect to make more than 10-15K per two years from the site.
