years back when the dunes was the high stakes joint in vegas their chip racks held 21 chips. this way when a crap shooter was winning big the boxman gave him racks to cash out and change to bigger denomination chips. so if he had blacks he lost 500 each rack.
the poker players and regulars knew to not fill a rack. many others got taken and even at the cage.
so things like this do happen on purpose.
