Had the same sort of thing in a bar league game (but this was the 'grand championship') where the players played 40 minutes (2 levels) without the Q of H because there was not one player at any table (which included some off-duty dealers at the local casino) who counted down the deck before they dealt. It was face up under the table.
I got pretty fast at doing it and had seen some sneaky stuff before so I just did it every second deal I did. I was not at the table missing the Queen for the record.
This is also the bar league I announced action as it was occurring and would often be looked at weirdly by the more experienced casino dealers who always sat there like rocks.
I think this was a by-product of it being all limit at my local casino, but it may just be some bad habits.
