As requested by Joey for the "preferred" method to submit glitch removal requests - I sent the following direct PM with detailed information.

I have seen this happen when the other players in the hand all limp preflop. I am uncertain if it happens if other players raise >1BB preflop. However the condition that triggers this glitch is the person sitting in the BB having less than 1 Big Blind and is automatically all-in.
Jaredpoker has commented about this issue months ago. I have also experienced this glitch several times too. It is still definitely not fixed.

IF any significant amount of effort ever went into resolving this issue.
It raises the question of how capable and coordinated the GlobalPoker/VGN team is behind the scenes.
