1. If you are absolutely certain which is 4th card that would have been the burn, then declare it the burn. Place it face up on top of the stub and burn it when the flop betting is complete. If not certain, mix the 4 cards face down, pick one to be the burn and place it face up on top of the stub, reveal the flop cards.
2. Premature exposed cards don't automatically play. Make sure players who haven't acted know the care won't necessarily play, finish the action. If it's the turn card, set that card aside, then burn and deal what would have been the river card. If you then need a river, shuffle the premature turn in and deal it without a burn. If the premature exposed card was the river, after action is complete, it gets shuffled in and dealt without a burn.
