At the 20/40 OE game at Bicycle, the winner would only post $20, but they called it an "action button", not a kill, and the limits stayed the same. Also the low card brought it in as usual, and players between the low card and the action button could only limp, not raise.
Personally I would recommend just not doing a kill in a stud game. As you said, even a half kill drastically increases the stakes of the game. In a normal 20/40 hand with a typical one raise (a complete), it costs $20 to see 4th street. If there is a half kill with a single raise, it now costs $60, or 3 times as much. If no one wants to raise, it actually costs 6 times as much as usual.
