I was never asked to verify my account when I initially deposited onto Lock. It took 4 separate tries to deposit with Visa. My 1st attempt was for $600 (unsuccessful), 2nd attempt for $600 (unsuccessful), 3rd attempt for $500 (unsuccessful), then finally 4th attempt with Visa for $400 (Success!).
I sent a PM to Rizen and email to Lock support, who both informed me of the verification requirement. So that's what I'm working on now. I will try the file upload process, with a separate email to lock support informing them. Good idea. I'll update with the results.
