Just had this hand, really unsure about whether or not this has to be a river call. Was in a 6x and this hand was vs a weak reg.

Don't fold. He's Def jamming quite a bit worse for value. He's very unlikely to have higher flush since most A/Kx hands don't limp pre. Trips, straights, and even some 2 pairs are likely to jam here. Given: those hands would probably jam turn, so I can imagine this could potentially be a spot to hero fold.

Have to call here, you're ahead of some of his value jamming range.

    Whaaat?

    This fold river hurt me.

I would call turn and shove river. As played call shove (he can shove less good hands)

Is this a PLO hand in disguise and I am missing something?

I must have been watching too many PLO vids on youtube haha

Over thought the spot but I still wonder what he really shoves there for value. Is he really shoving trips? I doubt he shoves two pair.
