This is a hand that happened to a friend of mine:
After a board of 3

Hero shows down 99.
Villain turns over his hand showing a lone 5 and throws his cards toward the pot, still with only the top card showing.
Dealer then separates his cards to reveal he had 5 7, and made a backdoor flush.
Villain was genuinely surprised and was then awarded the pot.
When hero complained, the dealer said she was just doing her job.
Floor sided with the dealer.
