Everyone's different but for me the root cause of this sort of play (still happens to me occasionally) is this:
- I start playing looser and looser because I'm card dead and the game is very loose which sometimes leads to...
- missing tons of flops leading to...
- bleeding chips steadily which starts...
- two forms of tilt at once.
1) "get-even" tilt (impatience, looser play, taking dumb luck personally, taking opponents' plays personally)
2) "I'm owed a winning hand" tilt (forcing situations, getting sticky, chasing, refusing to acknowledge solid reads "because I've got to win one soon - I've lost for hours!")
For me I know what to do I just sometimes don't know why I'm not doing it till I take a break, cool off and then think about whether I was just tilting. It'll be different for you but remember to look at your mood/psychology in the moment as well as your knowledge base and skills.
