tt already does this, just not with intertops. for example, if im playing bovada, and browsing the web, using skype, or whatever at the same time, when action is needed on a table, that bovada table will pop to the front of ALL windows, regardless of app. this is ideal (at least for me) as i can fold/shove/raise with a single click, without having to move the mouse to the table and click on it to gain focus first. every hotkey program ive ever used has done this - stars ahk, tableninja, merge keys, etc., altho actually i think this is a function of the poker client as i believe most clients will do this without any hotkey programs running. To avoid problems, i just dont use my hotkeys in other apps while im playing.

as far as layout goes, i either play with tiling mode disabled or on Fill, as i like to see all my tables.
again, all the other hotkey programs ive ever used haven't had a problem with new tables stealing focus from tables that already popped up needing action. granted, those programs were for a single poker site, but i believe ive had this problem playing just on intertops with tt.
I've also had severe lag issues, even one tabling (I see you posted a fix, I'll try), and also when minimised tt goes to the taskbar notification area, but after a while sometimes disappears and the only way i can access it again is to open the program again, which opens the tt home window showing that it is not currently running, BUT IT IS RUNNING. At that point, the only solution ive found is to open the task manager and close the process, then reopen the program.
