This hasn't been reported yet that I can remember. However, when I was developing this feature, I do remember running into the issue a few times. I specifically added code to make sure that SaT waits until the left click is released to try to fix it.
But when this happens, you are definitely clicking and releasing with your mouse, right? So SaT is correct in detecting the click? Its just that for some reason the poker site isn't responding to it?
ACTUALLY, I just tested it, and this is probably the problem:
It seems that SaT captures the click on the mouse DOWN, but doesn't stack the table until the mouse UP. So, if you click down on the Fold button, but hold the click and then move your mouse off the fold button and release, then of course the poker site won't click Fold because you've released the click outside of the Fold button. But SaT captured the click on the downpress, so it thinks that Fold was actually clicked.
Instead, SaT should probably wait to capture the click on the up-press. But
I believe I didn't originally do this because I ran into issues. I will need to revisit this. Thanks for reporting it.

Other 2p2 users ChAAnt JC has requested a feature to allow numpad to be sent to the betbox. Instead I will likely add an option to try to keep the betbox always focused, so this will probably help your problem once I develop it
He said that Stars doesn't automatically focus the betbox, and thats why this feature is needed. So you probably don't notice the problem on Stars because you always have to manually click the betbox anyway. But please see the previous post before yours, where I posted that Stars does infact have a "highlight betbox" option, but it seems to only work when its own "popup table when action required" is also turned on. Please confirm this.
As for 888 and Party, I don't know if their betboxes are supposed to be focused automatically by the site.
Perhaps what you're noticing is that the entire table is losing focus. This is probably more likely. You can test this by turning on the "Highlight active window" setting in Advanced Options
