Support Ticket with a
PokerTracker4.log file that's recorded the trouble so that we can investigate that for you along with a sample of relevant hand history files.

Most irritating thing is that sometimes when it moves those files, iPoker's client creates new empty note file, which then again gets moved and replaces the original note file in processed folder. In this case you have to search for some older backup player_notes.xml which doesn't have the all the newest notes.
Please check your import folder settings for iPoker. PokerTracker 4 should be configured to import from this folder:
C:\Users\{Windows Login}\AppData\Local\{iPoker Client Name}\data\{iPoker Screenname}\History\Data
As far as I am aware the iPoker player notes file is currently stored in:
C:\Users\{Windows Login}\AppData\Local\{iPoker Client Name}\data\{iPoker Screenname}
which suggests you are auto-importing from this folder (or higher). When PokerTracker 4 is set to auto-import from a folder it will read that folder and all of its subfolders so if it's not set at the correct level then that can cause problems.
