I think the key is how the final scene switches back and forth from Tony's viewpoint and the regular shot. When the screen goes black, it's Tony's viewpoint. He's looking up, expecting to see Meadow walk in. Another key part is during Tony's viewpoint, he's focusing on AJ. Meanwhile in the backround, you clearly see the dude at the counter staring at Tony. Tony is not paying attention to the guy. Then during the regular screen shot, you can see the guy is clearly nervous when he walks to the bathroom. If he's not there to kill Tony, why is he staring at him and being a nervous nellie?
But the 2 biggest things that foreshadow his death is Bobby talking about how you never see it and Phil getting killed in front of his family. Once that happens, killing Tony in front of his family is fair game.
Also, NY knows they can work with Paulie if he's in charge. He almost left to join Johnny Sac's crew when he didn't think he was getting respect. W/ Tony, Silvio and Bobby out, they can control the NJ mob because they can control Paulie.
