I wonder if they go the individual form route and not the one sign effects all for permit purposes. I also assumed that any random that goes back to the house has to sign the form, but I feel I've seen faces blurred out at the new house(in a preview clip). On season 1 I know for sure some faces got blurred out.
Releases in clubs and bars are handled on an individual basis. You walk up to the people after they're done interacting with the cast and ask them to sign, write a description down of them and the approximate they were on camera. Or, if the conversation is fairly long, the audio guy will tell a PA when there is a lull in the conversation to go in and grab the release. If a person is persistently talking to the cast and refuses to sign a release they'll probably be asked to leave them alone.
The shows I've worked on have had a policy that guests can't come into the house/main production area without signing a release. I assume Jersey Shore is the same, but there might have been a production issue (lost release, person deemed way too drunk to sign, etc.) on the back end that made the editors blur someone out.
