Code is long out the window
The show has never been about a vigilante superhero killing evil people to fill the cracks in the justice system. That a handful of episodes, particularly the early ones which establish his character and code are like that, is a device.
His code violations are important because they are code violations, and they are usually pointed out as such. He is evolving with and against: don't risk getting caught, kill those who need to be put down, kill dispassionately, don't let anyone close, etc.
