Probably d4 and punish them for wasting tempos on non-developing moves. I came across this "opening" often enough that I let Komodo take a look at it. Both d4 and 0-0 are evaluated as +0.34 (depth=31). Obviously engine evaluations this early in the game are to be taken with a huge grain of salt. d4 looks natural enough though.
If they take on d4 you might follow up with c3 if you care more about interesting games than the best moves.

d4 exd c3 dxc 0-0 is my go to line against 3...h6

Against the other "I am scared of Ng5" line, namely 3...Be7

d4 exd c3 dxc Qd5 is fun.

It's not lost for black, but looks like it is on first glance, and I've had opponents resign in this position more than once. However black's pawn on c3 now kind of saves the day for him. Engine thinks white is better though.

Edit: I might add that when black finds the correct line, I've not always had the best of games, as my queenside pawns are nonexistent and castling kingside will often run into g-file shenanigans. Certainly fun games, though.

I'm reading that subreddit way too much, considering how AIDS it is. Is there an actually decent chess forum out there? I'm not looking for comparably 2005-2010 twoplustwo levels of awesome, as that's pretty much impossible, but something better than /r/chess has got to exist, no?
