It comes from the view that not understanding and understanding are dichotomic, thus there must be a treshold at which understanding starts. One gains insight. This was a popular view in psychology around the 1900s. In retrospect this is likely a much too simplistic view of things, but it is helpful in modelling and constructing methods for learning/teaching - thus the term insight has stuck.
In reality my guess is that (human) not understanding isn't that much different from (human) understanding; Someone who does not understand a crossword puzzle, is probably much more similar to a person who does, than to a rock.
