The answer to these questions is quite different for CET Casinos (Ballys/PHo/Caesars) vs. MGM casinos (Bellagio/Mirage/Aria).
Comp earned in any way at any CET casino nationwide can be used however you like at any other CET casino nationwide. You just have your Total Reward card swiped when you are paying the check/hotel bill etc. You don't need to get any sort of comp slip in advance. You comp balance will expire if you have no activity of any sort on your Total Rewards card for 6 months.
At most MGM casino, comps earned at a poker room can only be used at a handful of restaurants at that casino. At least at Bellagio, you need to see the poker room manager on duty and have them write you a paper slip for a predesignated amount. That's the only MGM casino I've ever actually used a comp, but my impression is it is similar at others. The expiration of these comps seems pretty arbitrary to me; it used to be your comps wouldn't expire if you played at Bellagio at least once a year, but I think they recently changed this policy and removed a lot of players' comp balances without notice.
It is unfortunate there are no CET poker room with game selection competitive with the big MGM rooms, since their comp system is so superior except for true high rollers.
