Cody didn't luck into his gameplan, it was instilled in him by his coaches and using his experience. He's a better striker than Dom, he moves more efficiently than Dom. He can defend Dom's takedown attempts. He hits
much harder than Dom.
Dom is a smart guy, but he failed to make any meaningful adjustments during that fight, due at least in part to the fact that he was getting hit in the head repeatedly.
Injuries are real too. Dom can claim he was at 100%, but it's pretty silly to think that a lifetime of serious injuries to key parts of his body haven't slowed him down a half step. It may be such that his body is just breaking down at 31 because it was never that durable to start with. His hands are notoriously fragile as well, which is an obvious determent in a striking match.
Dom might come up with a better game plan, and he might be able to execute it, but this wasn't a fluke that should have anyone favoring Dom in the rematch. This was a 25 minute exhibition put on by a younger, quicker, harder hitting fighter over a Legend. The king is dead, long live the king.
