FYI...this seems to be the best (read: most reliable) option for RPM. A few folks have confirmed, but you need to follow exact instructions. Specifically, what I've bolded below seems to have hung up a couple of people, and took funds a little while longer to get cleared for play through support.

Sign the form, [I forgot to ask to make sure it was sent in dollars but it was]

Give the guy $deposit + fee

He will give you a receipt with a MTCN Number on it, buy beer to celebrate, go home, type in MTCN number in the box on web-page and click submit.

BOOM load up poker site and play.
